If this is your first time in our office, please select the Initial Visit that best describes you. For those 13 and older, please select Adult Initial Visit. For those under the age of 13 please select Pediatric Initial Visit. For a new practice member who is currently pregnant, please select Pregnancy Initial Visit.

Cancellation policy: If you no call, no show your initial appointment, you will be charged the full price.

Pricing update: All initial visits will be $85 starting July 1st no matter when your initial visit was scheduled.
